Community User Government Document Request

This form may be used if you do not have a library card from one of the following institutions:

Wright State and Affiliated Hospital Libraries
OhioLINK member institutions
The State Library of Ohio
Greene County Public Library or other SearchOhio Library

This form may ONLY be used to request government documents. If available, the document will be held at the Dunbar Library Circulation Desk for in-library use. Allow a minimum of three working days for the document to arrive. Call the circulation desk at (937) 775-2525 to check on the status of your request or provide an email address to receive notification when the item has arrived. It will be held no more than 9 days from the date of receipt.
